Serving Seniors Inc. announced that its Patient Advocate Program has been awarded a $5,000 Critical Needs Grant from the Scranton Area Community Foundation in the fall 2024 cycle. This support will enable Serving Seniors to continue its mission of providing personalized advocacy and support for older adults in Northeast Pennsylvania. The Patient Advocate Program addresses critical needs for seniors being discharged from local hospitals, assisting them in navigating complex health care systems, accessing resources, and helping them recover at home and meet their discharge goals. The program has been instrumental in improving health outcomes and reducing stress for seniors and their families in Lackawanna County.
