Serving Seniors Inc. announced that its Patient Advocate Program has been awarded $4,000 from the Robert H. Spitz Foundation, administered by the Scranton Area Community Foundation. This vital support will enable Serving Seniors Inc. to continue its mission of providing personalized advocacy and support for older adults in Northeast Pennsylvania. The Patient Advocate Program addresses critical needs for seniors being discharged from local hospitals, assisting them in navigating complex health care systems, accessing resources, and helping them recover at home and meet their discharge goals. The program has been instrumental in improving health outcomes and reducing stress for seniors and their families in Lackawanna County, officials said.
